- Returning clients may be required to complete updated forms if their health status changes.
- Participation in any service is voluntary. You may stop a treatment at any time by notifying a K5 Rejuvenation team member
- For your safety, please follow all instructions provided by K5 Rejuvenation staff before, during, and after your session.

- Appointments & CancellationsPlease arrive at least 10 minutes prior to your scheduled appointment. Appointments canceled with less than 24 hours’ notice, missed appointments, or late arrivals may be subject to cancellation fees or shortened sessions.
- Memberships & PackagesMemberships and prepaid packages are subject to their individual terms and conditions. Benefits, expiration dates, billing, freezes, and cancellation policies are outlined in the applicable membership agreement.
